Northwest Community Care Network (NCCN) is a network of primary care providers committed to delivering excellent medical care for the Medicaid population in northwest North Carolina.
Overview
Medicaid managed care in North Carolina consists of three initiatives: the Carolina Access Program, Access II and III Community Care Networks, and the HMO Program. Since April 1991, North Carolina has operated the Carolina Access Program, a Medicaid primary care management program currently operating in every county of the State. Carolina Access was developed to link recipients with local primary care providers, who assumed the responsibility for guiding the patient through the healthcare system and assured access to primary care. The HMO Program is the smallest of the three initiatives and is primarily in the Mecklenburg County area.
Access II and III Community Care Networks, established in 1998, is the most recent option available under Medicaid managed care. This initiative builds on Carolina Access and assists local providers in developing managed care delivery systems that coordinate a continuum of health care. Primary care providers are given the opportunity to work together with other community providers (physicians, hospitals, health departments, and departments of social services) and care managers to develop the tools, information and support needed to meet the healthcare needs of Medicaid recipients. The program is jointly administered by the North Carolina Office of Research, Demonstration, and Rural Health Development and the Division of Medical Assistance (DMA).
Northwest Community Care Network (NCCN) is one of 14 community care networks in the state with over 100 primary care practices, 500 primary care practitioners, and approximately 72,000 Medicaid enrollees. The network began in 1998 with Surry County as the original pilot site. NCCN began to add additional counties in December 2001 and has grown to cover Carolina Access recipients in six counties (Davie, Forsyth, Stokes, Surry, Wilkes, and Yadkin). With a focus on improving communication through information systems and on identifying health care system barriers, medical providers, and community resources, providers are working to enhance patients’ health and self-care capabilities across the health care continuum. The care managers place special emphasis on patient and provider education to help meet the goals of improving the access and quality of care while providing the most cost effective care.
NCCN areas of focus:
- Local care managers who work closely with provider practices, enrollees, and local community organizations.
- A system-wide education effort to help patients select the best place to receive care based on their present health status.
- Utilization of approved disease management protocols in an effort to optimize patient outcomes and reduce unnecessary hospital admissions and emergency room encounters.
