Northwest Community Care Network (NCCN) is one of 14 Community Care of NC networks in the state.  NCCN covers Carolina Access recipients in Davie, Forsyth, Stokes, Surry, Wilkes, and Yadkin counties.  By establishing regional networks, the program is establishing the local systems that are needed to achieve long-term quality, cost, access and utilization objectives in the management of care for Medicaid recipients.
 


As an organized community network of health care providers and care managers, we strive to improve the health and well being of our patients in an appropriate and cost effective manner.
